The application form requires that the patient’s cancer care physician sign the document. While obtaining the physician’s signature it is recommended that the patient discuss the therapy he/she is considering and ask any questions he/she may have. The physician may know about the therapy and be able to advise on its safety, use, and effectiveness.

If you are awarded a grant from our Foundation, the provider(s) on the application form will be contacted. The Foundation, not the cancer patient, will talk directly to the provider(s) and work with them to obtain the necessary documents for payment approval.

All service providers listed on the application form must be approved by the Sandra J. Wing Healing Therapies Foundation (Foundation), prior to any grant funds being released.

The patient will receive a phone call from one of the Foundation’s representatives regarding his/her service provider(s) approval.
